The fourth of ten thematic workshops was held in Viljandi, Estonia from the 14th to the 15th of June. The overall topic of the workshop was Health Risk Assessment, hosted and organized by the Viljandi Hospital Estonia in close cooperation with experts from the Catalan original Good Practice. The event was organized in a hybrid format (face-to-face and online) in which 24 people from different organisations participated.
After the welcome address from Dr. Mart Kull from the Viljandi Hospital, the Estonian Ministry of Social Affairs/Estonian Health Insurance Fund, and Josep Roca from JADECARE_EU, different sessions were held: “Financing care pathways and population health management –the future of value-based purchasing in Estonia” –Tiina Sats, Estonian Health Insurance Fund” “Potential and practicalities of the implementation of GMA and QUERALT indices –Analysis based on available information from NA.” David Monterde, Catalan Institute of Health” “Session on Estonian care-management pilots overview and tools to facilitate future implementation (Estonian Enhanced Care Management, Vilijandi PAIK project, Estonian Care Plan Software)”
The first day closed with a site visit at Viljandi “new hospital & primary care centrum”. The main goal of the second day was the reflection of Viljandi and Marche on the risk assessment results of the first PDSA (Plan Do Study Act) period, sharing some complementarities and consolidating an overview of Local Action Plans for the second PDSA period. Overall, the workshop was a fruitful exchange and excellent preparation for further implementation steps.
