The thematic workshop on Risk Stratification was held in a hybrid format in Aalborg, Region North Denmark (RND) from 20th to 21st of June in close collaboration with the original Good Practice model from the Basque Country in Spain: Basque health strategy on ageing and chronicity: Integrated care.
Approx. 15 people were attending in person, 15 online. While Next Adopters from Denmark, Spain, Italy, Estonia, Greece, and Portugal joined the thematic workshop on-site, one Next Adopter from Italy and approximately 15 attendees participates online.
In breakout sessions, Next Adopters elaborated implementation experiences on bottlenecks, barriers, facilitators, challenges, and learnings. The first day ends with a city walk with a focus on “Aalborg Development from an industrial town to a knowledge city with a focus on the waterfront, healthy outdoor areas and quality of life”.
- The results from day 1 build the basis for the work session on day 2 where mixed groups between Next Adopters and experts from Instituto de Investigación en Servicios de Salud Kronikgune (Basque original Good Practice-WP5) discuss findings from day 1.
- The second part focuses on how to strengthen the fitness of the implemented practice after the JADECARE project ends. After a wrap-up session and conclusions of the thematic workshop, the event closes with an overview of the next steps in JADECARE.
